Students seeking work permits must fill out these three documents. The student will need to print these documents to be filled out by themselves, parents, and new employers. After the forms are filled out, they can email them to terriann.thomas@perrylocal.org and she will process the work permit and email it back to the student to give to their employer. Any questions about work permits can also be sent to terriann.thomas@perrylocal.org.
